URS, IQ, OQ, PQ — and the re-qualification question

Equipment validation reads as a chain: a User Requirements Specification sets what the equipment must do, Installation Qualification evidences that it arrived and was set up correctly, Operation Qualification proves it behaves inside its specified envelope, and Performance Qualification shows it delivers the process outcome under real production conditions.

SAGA carries that chain as a single traceable thread. Each URS item binds to its qualifying phase and the acceptance criteria that close it; each IQ, OQ, and PQ protocol carries the URS references it answers; the signature trail reads cleanly from requirement to evidence without a cross-file reconciliation pass.

The harder question is not the initial qualification — it is the re-qualification trigger. A process change, a spare-parts swap, a cleaning-procedure revision: each might re-open IQ, OQ, or PQ, or none of them. SAGA records the change rationale against the original qualification so the risk-based decision is auditable, not reconstructed from memory.

The four phases, read in order

Each phase answers a distinct question, and each phase has its own acceptance shape. The discipline is keeping them separate on the way up and connected on the way down.

  1. User Requirements Specification. What the equipment must do, in process terms. Not "supports 200 L batch" — "delivers fill accuracy within tolerance at the 200 L process rate". The URS is the load-bearing document; every phase below binds back to its items.
  2. Installation Qualification. Evidence that the equipment arrived intact and was installed against specification. Serial numbers, utilities verification, calibration certificates, spare-parts inventory. IQ is a factual record: either every checklist line is closed or it is not.
  3. Operation Qualification. Evidence that the equipment behaves inside its specified envelope — temperature, pressure, flow rate, dwell time. OQ exercises the range, not the point; a qualified envelope is the equipment's operating charter for every batch that follows.
  4. Performance Qualification. Evidence that the equipment delivers the process outcome under real production conditions — batch-realistic loads, real materials, real operators, real cleaning cycles. PQ is where URS items close; once PQ passes, the equipment is in production service and every re-qualification trigger tracks back here.
